   POSITION:    SOCIAL WORKER - REHABILITATION

PERMANENT FULL TIME

 

 The Inpatient Rehabilitation program provides comprehensive multidisciplinary, goal oriented therapy for patients requiring intensive short-term rehabilitation due to disability or prolonged illness.  Care is focused on the attainment of realistic goals that will result in maximum functional improvement and facilitate a return to independent living in the community or a lower level of care in the community.

 

 QUALIFICIATIONS:

 

  • Current Registration with the Ontario College of Social Workers and Social Services workers.
  • BSW/MSW required.
  • Minimum three (3) years hospital experience preferred.
  • Demonstrated success with complex case management.
  • Ability to work and problem solve independently.
  • Strong interpersonal skills to function as a productive member of the multidisciplinary team.
  • Ability to organize and prioritize multiple demands.
  • Accurate and timely documentation, meeting professional and organization standards.
  • Demonstrated leadership skills.
  • Demonstrated commitment to own professional development and ongoing education.
