Unit Profile:
Pediatric Ambulatory Care is located in 3F, 2Q and 2F. There
are numerous subspecialty acute pediatric clinics within
McMaster Children?s Hospital. This Outpatient Clinic
provides acute tertiary and subspecialty medical, oncology
and surgical care to children aged 0-18 years. It is a
dynamic, fast-paced acute care unit where specialized care
is provided primarily for children in a family-centered
environment.
Position Summary:
The primary focus of this position is the pediatric
Digestive Diseases population. Through in-depth psychosocial
assessments, the Social Worker will identify child and
family strengths, stressors, problem-solving abilities,
dynamics, interactions, attitudes, feelings and cultural
issues in relation to the needs of the acute and chronic GI
patient and family, both in the outpatient and inpatient
settings. The Social Worker will work collaboratively with
the team to perform interdisciplinary assessments aimed at
developing and implementing a plan of intervention, which
addresses education, coping and compliance. This will be
achieved through a variety of modalities including
individual and family therapy, group work, consultation,
case management, crisis intervention, patient and family
education, community outreach and advocacy. The Social
Worker will identify and make referrals to appropriate
supports and resources and assist families in accessing
financial assistance programs when necessary.

Monday to Friday; days
Qualifications:
1. Master's Degree in Clinical Social Work from an
accredited School of Social Work
2. Current certificate of Registration with the Ontario
College of Social Workers and Social Service Workers
3. Minimum 3 years recent experience with children and
families in an ambulatory tertiary care pediatric setting
4. Minimum 3 years recent clinical experience providing
individual and family therapy for children and families
within a pediatric health care setting
5. Familiarity with the challenges facing children and
families managing digestive disease related disorders
6. Knowledge of adolescent transition to adult care for
individuals managing a chronic illness
7. Knowledge of community resources and referral process
8. Familiarity with the Mental Health Act, Consent to
Treatment Act, Child and Family Services Act
9. Demonstrated expertise in assessment, counseling, crisis
intervention and advocacy skills in working with children,
adolescents and families coping with medical and/or mental
health issues
10. Excellent communication and teamwork skills
11. Demonstrated ability to work in a fast-paced, changing
environment
12. Demonstrated commitment to ongoing learning and the
ability to teach
13. Proven ability to organize, prioritize and problem-solve
effectively
